Support at Home
Empowering older Australians to stay active, independent, and living well at home.
Our Support at Home Exercise Physiology services are designed to help you maintain strength, balance, mobility, and confidence so you can keep doing the things you love — safely and independently.
Through personalised home-based programs, our university-qualified Exercise Physiologists work with you to:
-
Improve mobility, strength, and balance to reduce your risk of falls.
-
Manage chronic conditions such as arthritis, diabetes, heart disease, or osteoporosis.
-
Support rehabilitation after injury, surgery, or hospital stays.
-
Build confidence with everyday tasks like walking, stairs, or getting up from chairs.
-
Stay connected, active, and living life on your terms.
We provide services in your own home or at our clinic, depending on your needs and funding arrangement.
If you’re receiving services under the Support at Home program (previously the CHSP), we can work with your care coordinator, GP, or home care provider to make sure your exercise plan aligns with your overall health and independence goals.
